docs(structure): restore Gate B wording and harden pin

Browse Source 
Drop the Gate B parenthetical that was not in the frozen AGENTS table.
Require Phase 0 labels in both compose and DEPLOYMENT, and pin the full
client-authoritative math lock phrase.
