- Archived `thornhill-website` → `10_archive/` (self-declared archived 10-17).
- Relocated 4 orphan MOCs → `MOC/` (AI&LLM, Business, Development, MOC - Meta).
- Relocated `Video Stack/` (13 notes) → `04_projects/video-stack/` + new status.md.
- Removed 3 stray/0-byte files + empty drift dirs `02_areas`/`03_resources`/`03_templates`.
- Moved welcome note → Meta/, cleared stale status: inbox.
- Phase 2: taxonomy extended (~50 real tags registered; no note-level placeholder pollution).
- Phase 3: refreshed MOC/Index, MOC/Work, MOC/Learning; updated index.md.
- Phase 4: documented Agent-OS folders in vault-structure.md.
- Phase 5: report at `Meta/health-reports/2026-10-18 — Defrag Report.md`; logged in agent-log.

### Issues detected / decisions pending (Tony)
- Video Stack: keep as project or promote to area `20_Areas/Video Stack/`?
- Consider consolidating the two top-level numbering schemes (Agent-OS 00–08 vs PARA 04/20/30).
- Missing `status.md`: `hermes-newsroom`, `voicestudio`.
- Dangling links in pre-crew MOCs → suggest Librarian/Connector.
- One-off tag tail (#byte-dance, #change-amplification, …) → suggest `/tag-garden`.
- Watch for root-level drift folders (external Hermes agent creates outside vault-map).
